Reports Of Birth
The Report of Birth is a declaration of the birth of a child born abroad to Filipino parent(s).
The Embassy enjoins all Filipino citizens in Greece and in Cyprus to promptly register the birth of their children with the Philippine Embassy in order to have their children registered with the Office of the Civil Registrar General, National Statistics Office (NSO), in the Philippines.
REQUIREMENTS FOR REGISTRATION OF BIRTH
1. Birth Certificate in Greek and its Official English translation
2. Report of Birth to be accomplished by the child’s parents in quadruplicate. Forms may be obtained from the Embassy’s Consular Section or downloaded from this site.
3. Photocopy of the passport of the child’s father and mother
4. If parents were married in the Philippines:
Marriage Contract of Parents on Security Paper (SECPA) from the National Statistics Office (NSO) and duly authenticated by the Malacañang Legal Office and the Department of Foreign Affairs (DFA)
If parents were married in Greece or in Cyprus:
Marriage Contract and Report of Marriage duly registered at the Philippine Embassy, Athens, Greece
If the parents of the child are not married:
1. Birth Certificate in Greek and its Official English translation
2. Report of Birth to be accomplished by the child’s MOTHER in quadruplicate. Forms may be obtained from the Embassy’s Consular Section or downloaded from this site.
3. Photocopy of the passport of the child’s MOTHER
